REQUIREMENTS FOR FIRST TIME PASSPORT APPLICANTS

  1. Personal appearance is required
  2. Duly-accomplished application form
  3. NSO-authenticated Birth Certificate with receipt
  4. NSO-authenticated Marriage Contract with receipt (married women)
  5. NBI Clearance or Bapstismal Certificate
  6. Photocopy of valid ID (front and back)
  7. School form 137 or TOR
  8. Note: Your photo will be taken at the Department of Foreign Affairs Office

 

REQUIREMENTS FOR RENEWAL OF PASSPORT

  1. Personal appearance is required
  2. Duly-accomplished application form
  3. Present passport and photocopy inside and back cover
  4. Pages showing latest Bureau of Immigaration departure and arrival stamp
  5. Personal appearance of mother (illegitimate minor)
  6. Note: Your photo will be taken at the Department of Foreign Affairs Office

 

REQUIREMENTS FOR REPLACEMENT OF LOST PASSPORT

  1. Personal appearance is required
  2. If lost passport is still valid, please submit police report
  3. Submit same documents required for first-time passport applicants
  4. Note: Your photo will be taken at the Department of Foreign Affairs Office
